Brain Health Lab Panels & Dementia Blood Test
Our brain health lab panels, including a dementia blood test, provide one of the most thorough laboratory pictures of your physiology and cognitive risk available outside of a research setting: 130+ biomarkers, blood-based dementia markers, and neurotransmitter profiling, interpreted by a subspecialist neurologist.
Available at our St. Marys, Georgia clinic.
Why Standard Blood Work Misses the Brain
The blood work your primary care physician orders once a year was designed to screen for metabolic disease, organ dysfunction, and cardiovascular risk. It does that reasonably well. What it does not do—what it was never designed to do—is assess the health of your brain.
A standard annual physical runs 20 to 30 tests. None of them measure neurotransmitter balance, blood-based Alzheimer’s biomarkers, intracellular NAD+ levels, neural toxin burden, or the oxidative stress markers that correlate with neurodegeneration. Your doctor is not ignoring these things. The tests simply are not part of the standard panel, and most commercial laboratories do not run them.
Our three-tier lab system was built to fill that gap. The Foundation panel establishes a comprehensive metabolic, hormonal, and vascular baseline. The Advanced Brain Health panel adds the subspecialist markers that are specific to cognitive risk and neural function. And the Additional Options let you target specific concerns—biological age, autoimmunity, cancer screening, gut health—based on your history and goals.

What Your Results Tell Us
A reference range tells you whether a value is statistically normal. It does not tell you whether that value is contributing to the cognitive symptoms you walked in with. That distinction is the difference between flagging results and interpreting them.
Dr. Orr reviews every panel personally—not a medical assistant, not an algorithm. He reads each marker in the context of your full results, your history, your imaging, and your goals. A testosterone level that falls within the “normal” range may still be inadequate for optimal brain function. An inflammatory marker that a standard lab report marks as unremarkable may explain why your processing speed has declined. Context changes the clinical significance of almost every result.
After review, you receive a detailed findings consultation that translates your results into a treatment plan. Every recommendation corresponds to something measurable in your data. Nothing is prescribed empirically. If the labs indicate that NAD+ depletion, hormonal insufficiency, or neuroinflammation is driving your symptoms, those are the targets—and the labs provide a clear baseline against which we measure your response to treatment.
Who Orders These Panels
Optimization Seekers
Executives, physicians, attorneys, and entrepreneurs who want a genuine baseline of their cognitive and metabolic health before anything declines far enough to notice.
Family History of Dementia
Patients with a parent or sibling diagnosed with Alzheimer’s disease or another dementia who want objective risk assessment—APOE genotyping, p-Tau 217, amyloid ratios—rather than waiting for symptoms.
Post-TBI & Long COVID
Individuals recovering from traumatic brain injury or persistent post-COVID cognitive symptoms who need neuroinflammatory and neurotransmitter data to guide treatment.
After Substance Use
People rebuilding cognitive health after years of substance use, whose neurotransmitter systems, nutritional status, and hormonal balance have been disrupted. The labs identify what needs rebuilding.
IBHP Patients
Patients entering our Intensive Brain Health Program receive these panels as part of their diagnostic workup. The results drive every treatment decision during the three- to five-day intensive.

What to Expect
Consultation and test panel selection
Estimated time: 30 minutes
Dr. Orr reviews your symptoms, medical history, medications, and treatment goals to determine which laboratory panels are appropriate. Brain health panels may include inflammatory markers, metabolic panels, hormone levels, nutrient status, heavy metals, genetic markers, and specialized biomarkers such as amyloid or tau proteins.
Blood draw and specimen collection
Estimated time: 15 minutes
A phlebotomist draws the required blood samples. Most panels require a standard venipuncture. Some tests may require fasting beforehand, which Dr. Orr's office will communicate when you schedule your appointment. The draw typically takes 10 to 15 minutes.
Laboratory processing
Specimens are sent to certified reference laboratories. Turnaround times vary by test, typically ranging from a few days for standard metabolic panels to two to three weeks for specialized biomarkers or genetic analyses.
Results review with Dr. Orr
Estimated time: 30 minutes
Once results are in, Dr. Orr reviews them with you in a follow-up appointment. He explains what each value means in the context of your brain health, identifies any abnormalities that warrant intervention, and incorporates the findings into your treatment plan.
Frequently Asked Questions
How is this different from the blood work my primary care doctor orders?+
A standard physical checks 20 to 30 tests focused on general health. Our Foundation panel alone runs 130 tests, and the Advanced panel adds markers most labs do not offer, including dementia biomarkers, neurotransmitter metabolites, and NAD+ levels. Every result is interpreted by a neurologist, not just checked against a reference range.

A standard annual physical typically runs 20 to 30 tests focused on metabolic health, cholesterol, and organ function. Those panels were never designed to assess the brain. Our Foundation panel alone includes 130 tests, and the Advanced Brain Health panel adds 36 subspecialist markers that most commercial labs do not even offer — including blood-based dementia biomarkers (p-Tau 181, p-Tau 217, amyloid beta ratios), neurotransmitter metabolites, intracellular NAD+ levels, and neural toxin panels. More importantly, every result is interpreted by a subspecialist neurologist who understands what these markers mean for your cognitive trajectory, not just whether they fall inside a reference range.
Do I need to fast before the blood draw?+
Yes. We ask for a 12-hour fast. Water is fine, and we recommend staying well-hydrated. The blood draw itself takes about 30 to 45 minutes.

Can I order just the Advanced Brain Health panel without the Foundation?+
You can, but we recommend starting with the Foundation panel. The brain depends on your overall metabolic health, so without that baseline, the Advanced results lack important context.

You can, but we generally recommend starting with the Foundation panel. The brain does not operate in isolation from the rest of your physiology — hormonal deficiencies, inflammatory states, vascular risk factors, and nutritional gaps all affect cognitive function. Without that baseline, the Advanced Brain Health results lack important context. Most patients order both, and we price them accordingly.
Who orders these panels? Do I need a referral?+
No referral is needed. You can schedule directly by calling our office. Patients include executives seeking a cognitive baseline, people with family history of Alzheimer's, TBI patients, and anyone who wants a thorough picture of their biological health.

What actionable steps come from abnormal results?+
Every abnormal result maps to a specific treatment. High homocysteine means you need targeted B vitamins. Insulin resistance prompts a metabolic protocol. Elevated inflammation may call for IV glutathione. The point of the panel is to build a treatment plan from your specific deficits.

Every abnormal result maps to a specific intervention. Elevated homocysteine indicates impaired methylation that responds to targeted B-vitamin supplementation. Insulin resistance prompts a metabolic protocol including dietary modification. Elevated inflammatory cytokines may indicate a need for IV glutathione or targeted anti-inflammatory treatment. Mitochondrial dysfunction markers guide decisions about NAD+ or Niagen IV therapy. The point of the panel is not to generate numbers but to build a treatment protocol from the specific deficits each patient carries.
