How We Work

Why We Don’t Bill Insurance

The Neurogenesis Project blends the best of the Direct Care model and the Concierge model into a single neurology practice. You pay a posted price, you get your physician’s full attention, and many patients with commercial plans are reimbursed by their carrier afterward.

The short version

Consider the last time you saw a specialist. The appointment was probably scheduled months out, lasted about fifteen minutes, and ended with a test that required approval from someone you never met. That is what a neurology practice looks like when its real customer is the insurance company.

We organized this practice around a different customer. You contract directly with your neurologist, the same way you would with an attorney or an architect. The visit is an hour. The price is on the website. The imaging gets ordered when the physician believes it will change your care, and the results are explained to you by the doctor who ordered them.

We also kept the part of concierge medicine that patients value: direct access to the physician between visits, through our ongoing-care memberships. What we left out is the part where a retainer is charged on top of insurance billing. Here there is one price, and it is the whole price.

Because we are not in any network, we do not file claims. We do, however, give you a complete superbill after every service, and many of our patients with commercial, non-federal plans find that their carrier allows them to submit it for out-of-network reimbursement. Details are below, along with a service that will file the claim for you.

What This Means for You

Six things our patients notice first

A full hour with your neurologist

A new-patient consultation with Dr. Orr is sixty minutes, not fifteen. That is enough time to take a real history, examine you properly, review outside imaging, and explain what we found in language you can act on.

The price before you commit

Every consultation, procedure, imaging study, lab panel, and infusion is posted on this website at a flat price. You know the cost before you book, and there is no surprise bill weeks later.

Testing ordered on clinical judgment

When Dr. Orr believes a brain MRI, a PET scan, or a biomarker panel will change your care, it gets ordered. There is no prior authorization queue and no appeal process standing between the decision and the test.

Direct access between visits

Patients in our ongoing-care memberships reach their physician directly, by telehealth and secure message, with protocol adjustments made as results come in rather than at the next available appointment months out.

Care that is not limited to a code

Much of what we do, from NAD+ and Niagen infusions to neurocoaching and regenerative therapies, has no insurance billing code at all. Working outside the claims system is what makes it possible to offer these treatments under physician supervision.

Time to think

A neurologist who is not carrying a panel of several thousand patients can read your full record, correlate your imaging with your examination, and write a report you can take to any other doctor.

Two Models, One Practice

Direct Care, Concierge, and what we took from each

Direct care practices trade insurance billing for a flat fee and more time per patient. Concierge practices charge a retainer for access and still bill insurance for the care itself. We took the flat pricing from the first and the access from the second, and applied both to specialty neurology.

Getting Reimbursed

Your commercial plan may pay you back

We do not file claims, but you can. Many of our patients with non-federal, commercial plans find that their carrier allows them to submit for out-of-network reimbursement. Here is how it works.

01

Receive your superbill

After every visit or service we give you an itemized superbill with the diagnosis codes, procedure codes, our tax ID, and the physician's NPI. This is the document an insurer needs to process an out-of-network claim.

02

Check your plan

Reimbursement is available to many patients with commercial PPO or POS plans that include out-of-network benefits. Federal programs (Medicare, Medicaid, TRICARE, and VA benefits) do not reimburse for care received here. Most HMO plans do not reimburse out-of-network care either. If you are unsure, the member services number on your card can tell you whether your plan has out-of-network benefits and what your deductible is.

03

File the claim

You can mail or upload the superbill to your insurer directly, or use Reimbursify, a service built for exactly this. You photograph the superbill in their app, they file the claim, and the reimbursement check or deductible credit comes from your insurance company.

04

Use pre-tax dollars

Health savings accounts (HSA) and flexible spending accounts (FSA) can be used for the physician services we provide, whether or not you also file with your insurer.

Let Reimbursify file it for you

Reimbursify is an independent app that turns a photo of your superbill into a filed out-of-network claim and tracks it until your insurer pays. Most patients submit a claim in about a minute, and the first claim is free.

File a claim with Reimbursify

Reimbursify is a third-party service and is not affiliated with The Neurogenesis Project. Reimbursement decisions are made by your insurer and depend on your plan’s out-of-network benefits, deductible, and coinsurance. We cannot guarantee any particular outcome.

Questions patients ask

Do you accept insurance?

No. The Neurogenesis Project is a direct-care practice and does not bill insurance or participate in any insurance network. You contract directly with your physician, pay the posted price at the time of service, and receive a superbill you can submit to your own plan.

Will my insurance reimburse me?

Many of our patients with commercial, non-federal plans (most PPO and some POS plans) find that their carrier allows them to submit for out-of-network reimbursement. The amount depends on your plan's out-of-network deductible and coinsurance. Federal programs (Medicare, Medicaid, TRICARE, and VA benefits) do not reimburse for care received here, and most HMO plans do not reimburse out-of-network care. We provide the superbill; the decision rests with your insurer.

How do I file for reimbursement?

Submit the superbill to your insurer by mail or through their member portal, or use Reimbursify, which files the claim for you from a photo of the superbill and tracks it until it is paid.

Can I use my HSA or FSA?

Yes. Our services are physician services and qualify as medical expenses for HSA and FSA purposes.

Is financing available?

Yes. We offer in-house payment plans and third-party financing through Affirm. Each phase of a program can be financed on its own, so you can start with an evaluation and decide on the next step once you have results.

Why not just join an insurance network?

Because the things our patients value most, long visits, same-week imaging, direct access to their neurologist, and treatments that have no billing code, are the things a network contract removes first. We built the practice around the care, and the payment model follows from that.

See the prices, then decide

Every service is posted at a flat rate. A new-patient neurological consultation with Dr. Orr is $250 for a full hour, and you can book it online today.
