I’ve spent four decades in this work, watching the same brain pattern fail and recover in real patients. The salience network — the part of the brain that decides what matters — gets hijacked. Substances become the most salient cue. Everything else gets crowded out. That isn’t moral failure. It’s a clinical event, and it has a clinical path back.

Most rehab tries to argue the patient out of using. That work has its place, but it lands on a brain that isn’t physiologically ready to hear it. We start where the brain actually is: imaging, lab work, neurochemistry, neurohormones, peptides where they help. Then the cognitive and 12-step work lands on a brain that can take it in.
